3. Your contact information (name, address, phone, email).
4. A statement that you have a good-faith belief that use of the material is not authorized.
5. A statement, under penalty of perjury, that the information is accurate and you are authorized to act on behalf of the owner.
6. Your physical or electronic signature.

Upon receipt of a valid notice, we will remove the material promptly.

Counter-notice: If you believe your content was removed in error, reply with a counter-notice and we will restore it (unless the copyright owner files legal action).

Repeat infringers will be banned.
